Recovery Worksheets

These free addiction recovery worksheets are companions to CONNECTED Recovery’s nine-process peer-education program. Use them to reflect on choices, recognize patterns, and prepare one practical next step. You can work privately, at your own pace, or bring questions to a meeting. Sharing is always voluntary.

Start with the participant workbook

Download the full participant workbook (PDF) for explanations, examples, and guided exercises. Choose one exercise that fits where you are today; you do not have to finish everything at once. You can also review the nine-week syllabus (PDF) and nine principles (PDF).

Use the tools with support

Write only what you feel comfortable keeping. Pause if an exercise feels overwhelming, and seek qualified support when needed. The workbook includes CBT-informed reflection tools; using a worksheet is not the same as receiving CBT from a professional.

Explore our approach, read about connection in recovery, or join our Tuesday online meetings at 8 PM Eastern. The existing meeting section has current joining details.

CONNECTED Addiction Recovery is volunteer-facilitated peer education and mutual support, not professional treatment, diagnosis, therapy, medical advice, detoxification, or crisis care. We are independent of and not affiliated with AA or SMART Recovery. These resources do not replace qualified care.
